If you are 65 or older and have health problems, you may want to complete and display a File of Life Form.
What is it? It’s a simple form that puts your essential health information in one place so that first responders and medical personnel can get the information they need in order to take proper care of you in an emergency. It’s part of a totally voluntary national program that is designed to help people as they get older.

Where should it be displayed? Someplace prominent, like an entryway or a refrigerator door.
